Phases of the LCA Evolution Program

🔹 PHASE 0 – Preoperative

Recovery begins even before surgery.

We aim to arrive at the operating room with a knee that is ready.

This makes a big difference in evolution.

🔹 Phase 1 – Control and basics (First month)

It is the most critical phase after surgery.

The goal is not to "do a lot," but to do the right thing at the right time:

  • Pain and inflammation control.
  • Recovering the extension (key).
  • Progressive mobility.
  • Initial muscle activation.

This is where the foundations of the entire process are built.

🔹 Phase 2 – Activation and strength (Months 2-4)

The knee is starting to respond, but it's not ready yet.

Work is being done on:

  • Full recovery of range of motion.
  • Motor control and proprioception.
  • Normalization of gait.
  • Real strength development.
  • Progression in loads.
  • Align the biological phase with the functional phase.

It is the phase where many patients believe they are doing well… but there is still much to be done.

🔹 Phase 3 – Return to Train (Months 5-7)

We are entering a more demanding phase. Pay special attention during this phase and to following the process.

The work is evolving towards:

  • Strength in all its forms.
  • Unipodal control.
  • Jumps, landings and plyometrics.
  • Changes of direction.

Here we really start preparing the knee for sports.

🔹 Phase 4 – Return to Performance (Months 8-9)

We're not just looking to return.
We are looking to return safely.

In this phase:

  • Specific sports gestures are integrated.
  • Control and confidence are refined.
  • The risk of relapse is reduced.

It is the step prior to a full return to performance.
